    SUMMARY OF POSITION: This case manager will utilize wrap-around philosophy and techniques to assist clients in transitional housing. In doing so, the case manager will meet with clients to maintain family and natural supports in the community, collaborate with the clients and track the success of their Individual Service Strategy/Treatment Plan of Care, coordinate individualized services and the involvement of providers working in the home and community setting, and document all support related to activities and services. RESPONSIBILITIES: • Visit clients weekly to track Individual Service Strategy goals, updating as needed • Work with other case managers (internal and external) to address long-term housing and financial stability needs • Provide emotional and informational support for permanent housing by assessing the client’s strengths, desires, and immediate/long term needs • Conduct comprehensive, ongoing assessments of program participants mental illness, physical health, housing status and other relevant needs, where appropriate • Collect and maintain log sheets that document client all meeting attendance • Conduct client drug and breath testing as needed • Develop and/or contact family and natural supports which exist in the community • Receive input from team members regarding client employment, housing, mentoring, substance abuse, mental health and other immediate/long term needs • Complete daily and monthly departmental paperwork • Update clients’ electronic files daily to include any types of contact with clients, services, or any internal or external referrals made (within 24 hours of service provision) • Adhere to the strategies and goals outlined in the ISS and Treatment Plan • Advocate for clients while providing coaching to make solid, well-informed decisions to maintain housing • Assist client with monthly budgeting and provide financial/money management case management sessions • Complete ISS and treatment plans, track client goals weekly with client • Must possess a Missouri Class E Driver License (MO residents only) • Conduct and document 30 and 60 day follow-ups, and beyond on program placements • Provide client aftercare retention services for up to one year after program exit Perform Outreach by - • Referring clients to additional resources to address other service needs (i.e. legal assistance, VA benefits, community substance abuse counseling etc.) • Research additional benefits available for clients in efforts to enhance services being provided • Engage the client, the family, and natural supports within the community setting (when feasible and desired by client) -Any other duties as assigned– DIRECT REPORTS: • None KNOWLEDGE AND EXPERIENCE: • Bachelor’s degree required; Peer Support Certification, MSW or related field preferred • Ability to follow, understand and meet grant requirements, including managing paperwork and electronic documentation • At least one years’ experience working in supportive housing • Understanding of Harm Reduction and Housing First, with knowledge of housing and other supportive service resources • Ability to engage and work with community agencies and programs • Available to work a minimum of 37.5 hours a week and reliable transportation is required SKILLS AND ATTITUDES REQUIRED: • Interest in helping Substance Abuse/Recovery clients reach their full potential • Ability to work well with a wide range of people in various settings with a capability to respect ethnic and cultural diversity • Excellent computer skills and the ability to process large amounts of data • Openness to learning new approaches with a willingness to take on new tasks • Ability to work in a team setting • Ability to set and keep healthy boundaries and be a good role model for others • Ability to handle confidential information • Ability to remain calm in stressful situations and to manage intensive caseload • Embracing of St. Patrick’s mission and the dignity and worth of all persons RELATIONSHIPS REQUIRED: • Reports to GPD Director • Works with internal and external agency staff • Develops relationship(s) with client family and natural supports • Networks with federal, state and local agencies RESOURCES: • Caseworthy and ServicePoint computer tracking systems • Various programs throughout the SPC Partnership Center • Client records and reports • Client access to vouchers for transportation, clothing, food, furniture, and other needs • Other resources as needed and identified
