JOB TITLE: City Coordinator – SSVF AGENCY: St. Patrick Center DEPARTMENT: Employment REPORTS TO: Director – SSVF DATE OF REVIEW: July, 24, 2020 SUMMARY OF POSITION: The SSVF Coordinator will oversee the City portion of the SSVF program, monitor all progress, and engage both community partners and clients in the SSVF program. RESPONSIBILITIES: • Respond to calls and requests from the community to assist homeless persons when possible • Make persons in need of assistance aware of community resources, especially from St. Patrick Center’s SSVF program • Connect prospective clients with a case manager for an assessment for services • Research various social service agencies to understand outside intake procedures and criterion • Conduct regular visits to social service agencies to inform them of the SSVF program and criteria for referrals • Refer clients to appropriate community agencies, assisting clients when needed • Maintain appropriate documentation of all outreach provided to agencies and clients • Oversee paperwork and case management services provided to clients within your region • Record services provided in ECM and keep all paperwork updated (within 24 hours) • Attend local Continuum of Care meetings to inform agencies regarding SSVF program • Maintain weekly schedule of CM’s stations and needed service areas to be covered. • Assist with case management responsibilities as needed • Track & compile paperwork,(necessary forms required by VA) on a monthly basis, from all case managers • Detailed knowledge of the grant and SPC Policy & Procedure, to assist case managers with following grant guidelines • Network within the community for resources to aid veterans • Work to ensure STL ends veteran homelessness • Report to Director of SSVF at least weekly, and other times as needed • Supervise Case Managers within your region, provide technical, case management, and supervisory assistance to reports • Become a “super user” with HMIS system(s) to provide technical assistance for daily reporting • Run reports on a monthly basis to track spending and grant compliance • Conduct staff meetings when necessary • Must possess a Missouri Class E Driver License (MO residents only) • Other duties as assigned Direct Reports: • Case managers within their SSVF region KNOWLEDGE AND EXPERIENCE: • Bachelor’s degree required; BSW preferred • Veteran status preferred • Residence within the specified region preferred but not required • Previous experience and knowledge from working with the homeless • Experience working in a social service agency • Familiarity with the fields of mental illness and drug/alcohol abuse; symptoms, terminology, and interventions • Familiarity with appropriate and available community resources SKILLS AND ABILITIES: • Ability to recognize personal strengths and limitations regarding time, skill, and knowledge • Ability to communicate effectively with both clients, social service agencies, and team members • Ability to build productive relationships within the community and with prospective clients • Ability to set limits with clients • Ability to engage new clients • Ability to assess persons and situations RELATIONSHIPS REQUIRED: • Work as a team member • Communicate verbally and in writing to case managers, and SSVF Director on a daily/weekly basis • Maintain relationship with related agencies RESOURCES: • Distributes supplies in appropriate manner • Report vehicle problems to supervisor • Use credit card for van gasoline • Purchase supplies that are needed for van
