    SUMMARY OF POSITION: The Community Resource Coordinator will utilize wrap-around philosophy and techniques to assist clients who are transitioning from homelessness to self-sufficiency and stability in the community. This coordinator will assist the case managers and coordinate individualized services for housing placement, coordinate the involvement of providers working in the home and community setting, and document all support related to activities and services. This coordinator will assist the HERO manager with daily tasks and coordinate the case management team with daily tasks and duties. RESPONSIBILITIES: • Meet with clients weekly to assess needs, update treatment plans and address housing goals. • Complete the housing goal with the client within 2 weeks of program placement • Detailed knowledge of the grant and SPC Policy & Procedure, to assist case managers with following grant guidelines • Contribute to the ideas and processes of the ICM team • Assist with housing placement based on the desires of clients • Work with the ICM team to address long-term financial stability needs • Assess the clients’ strengths, desires, and immediate/long term needs • Receive input from ICM team members regarding employment, housing, mentoring, substance abuse, and other immediate/long term needs • Complete daily and monthly departmental paperwork • Update daily clients’ electronic files in ECM and include any types of contact with client, services, or any internal or external referrals made • Adhere to the strategies and goals outlined in the ISS and Treatment Plan • Assist team members in addressing the needs of the client by providing vouchers (e.g., transportation, clothing, furniture, food, etc.), hygiene kits, job referrals, or any other necessary referral (e.g., mentoring, substance abuse, mental health, advanced training opportunities, etc.) • Coach clients to make solid, well-informed decisions • Refer client to additional resources to address other service needs (i.e., legal assistance, VA benefits, community substance abuse counseling etc.) • Research additional benefits available to enhance service being provided • Engage the client, the family, and natural supports within the community setting (when feasible and desired by client) • Must possess a Missouri Class E Driver License (MO residents only) • Other duties as assigned DIRECT REPORTS: • None KNOWLEDGE AND EXPERIENCE: • Bachelor’s degree from an accredited university or college; BSW or related field preferred, or equivalent experience with proof of ability to perform effective case management • Knowledge of housing and other supportive service resources • Available to work a minimum of 37.5 hours a week • Reliable transportation is required • Knowledge of Housing First • Knowledge of coordinated entry • Knowledge of Harm Reduction • Trauma Informed Care Trained SKILLS AND ABILITIES: • Interest in helping Substance use/Recovery clients reach their full potential • Ability to work well with a wide range of people in various settings • Ability to follow, understand and meet grant requirements, including paperwork • Ability to engage and work with community agencies and programs • Excellent computer skills and the ability to process large amounts of data • Ability to handle confidential information • Ability to remain calm in stressful situations • Ability to manage a large caseload RELATIONSHIPS REQUIRED: • Reports to Manager – HERO • Works with other ICM staff • Develops relationship(s) with client family and natural supports • Works with other Permanent Supportive Housing Staff • Works with staff in other SPC programs • Networks with federal, state and local agencies
