    SUMMARY OF POSITION: This case manager will utilize wrap around services and techniques to assist clients who have successfully moved from homelessness to permanent housing. This case manager will work with St. Louis area Grant Per Diem (GPD) program clients who have successfully moved from transitional housing or homelessness to permanent housing for a period of 6 months. The case manager will work with Veteran service providers to ensure care continues after 6 months and ensure stability for the Veteran and continued success. RESPONSIBILITES: • For a period of 6 months after transition to permanent housing, visit each client weekly or as needed to continue service delivery and continue treatment plan goals. • Document each visit/attempt into Caseworthy • Provide support and resources as needed/available to aid clients in achieving treatment goals • Address the needs of the clients by providing vouchers (e.g. transportation, clothing, furniture, food, etc.), hygiene kits, job referrals, or any other necessary referral (e.g. mentoring, substance use, mental health, advanced training opportunities, etc.) • Coach clients to make solid, well-informed decisions • Asses the clients’ strengths, desires, and immediate/long-term needs • Develop or contact family and natural supports which exist in the community • Work with VA GPD Liaison on GPD case management referrals • Be able to travel to 7 surrounding counties including Warren, St. Charles, St. Francois, Franklin, Washington, Lincoln and Jefferson Counties • Collaborate with area GPD teams on client services and discuss clients treatment • Receive input from are GPD team members regarding employment, housing, mentoring, substance abuse, and other immediate/long-term needs • Complete daily and monthly departmental paperwork • Update daily clients’ electronic files in Caseworthy and include any types of contact with client, services, or any internal or external referrals made • Refer client to additional resources to address other service needs (i.e. legal assistance, VA benefits, community substance abuse counseling, etc.) • Research additional benefits available to enhance service being provided • Engage the client, the family and natural supports within the community setting (when feasible and desired by the clients) • Coordinate with Veteran service providers on client care • Must possess a Missouri Class E Driver License (MO residents only) • Other duties as assigned DIRECT REPORTS: • None KNOWLEDGE AND EXPERIENCE: • Bachelor’s degree from an accredited university or college; BSW or MSW preferred, or equivalent experience with proof of ability to perform effective case management • Trauma informed care trained • Knowledge of housing first • Knowledge of harm reduction • Knowledge of housing and other supportive service resources • Available to work a minimum of 37.5 hours per week • Reliable transportation is required SKILLS AND ATTITUDES: • Ability to follow, understand and meet grant requirements, including paperwork • Ability to engage and work with community agencies and programs • Interest in helping substance abuse/recover clients reach their full potential • Ability to work well with a wide range of people in various settings • Excellent computer skills • Ability to process large amounts of data • Ability to handle confidential information • Ability to remain calm in stressful situations • Ability to manage a large caseload RELATIONSHIPS REQUIRED: • Reports to Manager – HERO • Works with other ICM staff • Develops relationship(s) with client family and natural supports • Works with other Employment Department staff • Works with staff in other SPC programs • Networks with federal, state and local agencies
