    JOB TITLE: Case Manager, Rosati CCT AGENCY: Rosati Center REPORTS TO: Director, Rosati Center EFFECTIVE DATE: May 1, 2020 SUMMARY OF POSITIONS As part of the CCT multidisciplinary team, the CCT Case Manager will provide case management services to program clients with mental illness and/or co-occurring substance use history. The Case Manager will assess client needs regarding housing, apartment searches, budgeting, financial literacy, and overall case management support. The Case Manager will handle financial counseling components, employment/rehabilitation assistance, housing assistance, interagency and community referrals, and all other financial aspects to Permanent Supportive Housing. Specific Duties and Responsibilities • Conduct comprehensive, ongoing assessments of program clients’ mental illness, physical health, housing status, financial status, and all other relevant needs regarding wrap around services • Assist clients in seeking competitive employment, if applicable, and document progress • Refer to appropriate internal employment programs as applicable and external community resources and networks • Provide financial and money management counseling and document progress as applicable • Assist clients with debt assistance and budgeting as applicable • Work with client’s regarding their benefits, not limited to SSI, SSDI, and other government entitlements and help clients obtain needed documentation regarding their housing • Assist clients with maintaining and keeping appointments, as applicable • Provide transportation to clients for client appointments, on an as needed basis • Provide direct clinical services to clients on an individual and group basis, in office and community, while making appropriate changes to treatment plans and other client goals and progress. • Complete client outcomes at a 30, 60, 90 day plan • Complete Home visits with all community and on-site clients at a minimum of one time per month and as needed for collaboration within providing services. • Complete all required documentation forms on client case notes in Case worthy and all other SPC/Grant required documentation portals • Collaborate with team members for staff meetings and individual supervision • Provide on-call crisis intervention in coordination with CCT on-call policy • Maintain client tracking systems, client files, while documenting progress to maintain client record • Maintain a positive relationship with each client • Be familiar with each client’s diagnosis, medications and history in order to assist in the implementation of each client’s treatment goals • Assist in creating a comfortable atmosphere for clients • Treat each client with respect, keeping good professional boundaries • Communicate regularly with supervisor for performance and feedback, as well as special assignments • Perform any and all miscellaneous tasks as required or requested by Supervisor KNOWLEDGE AND EXPERIENCE: • MSW, or MA Counseling preferred • Current Med Tech Certificate • Current proof of First aid training and CPR • Current physical/ TB screen • Basic Microsoft Office skills, required (Word, Outlook, Excel) • Understanding of mental illness and substance use • Experience in case management • High level of confidentiality, sound judgment and discretion • Excellent written and verbal communication skills • Solid computer skills • Must be detail oriented, accurate and reliable • Ability to multi task in a fast-paced team setting, in addition to the ability to work independently • Knowledge/understanding of area/community resources, to provide appropriate referrals • Knowledge of Federal, State, and Agency policies of HIPPA confidentiality.
