    JOB TITLE: Weekend Case Manager- Rosati Group Home AGENCY: Rosati Center REPORTS TO: Director- Rosati Center EFFECTIVE DATE: June 1, 2020 SUMMARY OF POSITIONS Specific Duties and Responsibilities Assist in implementing the daily direct care of all clients/residents in compliance with treatment and program goals for Rosati Center Group Home. • Maintain a positive relationship with each resident • Be familiar with each resident’s diagnosis, medications and history in order to assist in the implementation of each client’s treatment goals • Conduct comprehensive, ongoing assessments of all resident’s mental illness, substance use, physical health, housing status, and other relative needs • Provide direct clinical services to all residents on an individual and group basis in the office and community settings, making appropriate changes to treatment plans to ensure the client’s abilities and resources • Assist in creating a comfortable atmosphere for residents • Treat each resident with respect, keeping good professional boundaries • Report behavioral/physical changes, problems, an or concerns regarding residents to daytime Social Worker and respond/intervene as needed in a therapeutic manner • Maintain Group home client case notes through BJC requirements and Case worthy • Direct incoming calls in a timely and professional manner at the Front desk • Provide crisis intervention when necessary • Communicate regularly with supervisor for performance and feedback, as well as special assignments • Perform any and all miscellaneous tasks as required or requested by Supervisor, Social Worker, and/or management • Other miscellaneous and special projects as assigned KNOWLEDGE AND EXPERIENCE: • MA degree in Social Work or Counseling required • Understanding of mental illness and substance use • Experience in counseling and case management • High level of confidentiality, sound judgment and discretion • Excellent written and verbal communication skills • Solid computer skills • Must be detail oriented, accurate and reliable • Ability to multi task in a fast-paced team setting, in addition to the ability to work independently • Knowledge/understanding of area/community resources, to provide appropriate referrals • Knowledge of Federal, State, and Agency policies of HIPPA confidentiality. SKILLS AND ATTITUDES: • Good communication skills • Well versed regarding ethical issues • Knowledge of counseling and crisis intervention skills • Self-motivated • Respectful to peers and clients • Sensitive to cultural diversity • Ability to work in a team environment • Moderate computer skills • Ethical/knowledge of confidentiality RELATIONSHIPS REQUIRED: • Report to Director- Rosati Center • Attend and participate in all appropriate SPC meetings
