St. Thomas Community Health Center is growing!! We are looking to add a HIV Pre-Exposure Prophylaxis (PrEP) Navigator.

The PrEP Navigator will assist members of a priority population, who are at substantial risk for acquiringHIV, gain access to PrEP.

Job description:

The role will include, but is not limited to, the following:The PrEP Navigator is responsible for conducting outreach and in-reach activities, assist withintake and provide education to potential PrEP patients seeking services. The PrEP Navigatorwill also assist in the coordination of medical appointments, troubleshoot patient insuranceissues, enter data into electronic health record, conduct follow up with clients, and provideretention and adherence counseling to PrEP patients.

Education

Required - High School diploma or equivalent

Bachelors or masters degree highly preferred.

Work Experience

Required - 2 years of experience related to HIVand/or PrEP.

The ability to maintain patient confidentiality is a must.
