    Job Summary

    Core Duties:

    Works collaboratively with multidisciplinary teams to optimize drug therapy.

    Determines type of substance, strength, dosage, and quantity to be dispensed in accordance with accepted

    practice and pharmacy standing operating procedures (SOPs).

    Receives and reviews prescriptions for the full range of pharmaceuticals to include controlled drugs on the

    formulary.

    Performs advanced pharmaceutical calculations in the manufacture of intravenous admixtures and complex

    parental nutrition solutions dealing with body surface area and creatinine clearance rate formulas.

    Obtains the pharmaceutical, prepares package labels, visually inspects the medication in the bottle to be

    dispensed, and verifies appropriate medication, checks the printed label affixed to the container against the

    original prescription for correctness.

    Reviews and interprets medication orders to inpatients to ensure dosage, route, frequency, potential

    interactions, and allergies have been screened.

    Performs medication distributive functions, including, but not limited to, preparation and/or verification of

    medications, labeling, compounding, and dispensing.

    Follows established procedures and protocols concerning quality assurance monitoring, security of controlled

    substances, and disposal of hazardous medications.

    Participates in orientation, instruction and training of pharmacy technicians, residents, pharmacists, and other

    health professionals.

    May be identified per shift as a Code Responder and will respond to Code Blues during a particular shift.

    The Contractor shall be available to provide support (Monday through Friday) from (0800-1700) daily. Contract

    Personnel shall be available to provide support (Sunday through Saturday) from (0001-2400) daily.

    The Department shall establish a schedule for contract personnel to perform services on various shifts, which range

    from (8-12) hours. Each shift shall include (30 or 60) minutes for unpaid lunch, which may vary depending on the section

    and/or specific shift assigned.

    Regular duty hours are expected to average (40) hours per week. Unplanned or unannounced schedule changes may

    occur, based on necessity, and in concurrence of the Contractor.

    All pharmacists can rotate between any ambulatory pharmacy or inpatient pharmacy sites to accommodate staffing

    needs.

    Education:

    Degree/Education : Bachelor's degree in pharmacy. Graduate from a college or university accredited by the

    Accreditation Council on Pharmaceutical Education (ACPE).

    Equivalency Determination Request: The Contractor shall not request an equivalency determination as a

    substitute for education requirements on this labor category.

    Certifications (in addition to Basic Life Support identified in paragraph 1.5.45.6.):

    Licensure :. Current, full, active, and unrestricted license to practice as a pharmacist.

    Experience : A minimum of one (1) year of inpatient pharmacy experience within the past two (2) years post-

    graduation.
