About the Role:

Skilled Nursing Pharmacy in San Diego, CA is looking for a dedicated and knowledgeable Staff Pharmacist to join our team. This is an exciting opportunity to make a meaningful impact by providing expert pharmaceutical care to skilled nursing facility patients who depend on precise, compassionate medication management every day.

Responsibilities:

Dispense and verify medications accurately for skilled nursing facility (SNF) residents

Perform drug utilization reviews (DUR) and ensure medication therapy appropriateness

Collaborate with nursing staff, physicians, and care teams to optimize patient outcomes

Monitor for drug interactions, contraindications, and adverse drug reactions

Ensure compliance with state and federal pharmacy regulations and long-term care (LTC) guidelines

Manage controlled substance handling, documentation, and reporting

Support medication administration record (MAR) accuracy and pharmacist order review

Requirements:

Active California Pharmacist License (RPh) in good standing

Doctor of Pharmacy (PharmD) degree from an accredited institution

Experience in long-term care, skilled nursing, or institutional pharmacy preferred

Strong knowledge of geriatric pharmacology and SNF regulatory requirements

Familiarity with pharmacy dispensing software and electronic health records (EHR)

Excellent attention to detail and strong clinical judgment

Effective communication and team collaboration skills
