  • Responsibilities

    ASAP SERVICES HOME HEALTHCARE AGENCY is looking for a Licensed Practical Nurse LPN to assume responsibility and accountability for the application of the nursing process and the delivery of in-home patient care, under the supervision of a Registered Nurse (RN).

    RESPONSIBILITIES

    Assesses patient's physical, mental, and emotional status and changes in patient's support system. Assesses patient's and family's knowledge of health problems. Observes, record, and report changes in patient status to the case manager.

    Contributes to the achievement of established goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Completes annual Code of Conduct, Compliance, and other mandatory training. Complies with governmental and accreditation regulations.

    Maintains effective working relationships with other departments and participates in multidisciplinary quality and service improvement teams. Participates in meetings and on committees and represents the department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.

    Participates in care planning and development of patient goals.

    Participates in the implementation of care plan interventions, including teaching. Under the guidance of the case manager, provides direct nursing care according to physician's plan of treatment; administering selected treatments and medication, and performing procedures in accordance to agency standards of practice. Provides for emotional support and physical comfort and safety of patients. Assists with the rehabilitation of patients and encourages independence in activities of daily living according to patients' capabilities and potential.

    Performs other duties as assigned.

    Under the guidance of the case manager, evaluates patient and family response to the plan of care and effectiveness of teaching. Documents patient/family progress toward goals and outcomes according to agency clinical record policy.

    REQUIREMENTS

    • Current Licensed Practical Nurse (LPN) License in the District of Columbia.
    • Two years of recent clinical experience in an acute care setting is required; home care experience is preferred.
    • Current CPR Certification.
    • Verbal and written communication skills. Basic computer skills preferred.
    • Ability to work flexible hours.
    • Driver's License.
    • Clear Background Check and Drug Screen.
    • Must be fully vaccinated or be willing to complete full vaccination by date of hire and proof of vaccination will be required
