We’re currently seeking an experienced and dedicated Start of Care Registered Nurse (RN) to complete Start of Care (SOC) visits for our home health clients in the Greater Springfield area. Key Responsibilities:
As a Start of Care RN, you will be the first clinical point of contact for new home health patients. You’ll ensure every client receives a thorough assessment, a safe and personalized care plan, and a smooth transition into ongoing home care services.
Conduct comprehensive Start of Care (SOC) assessments, including physical, psychosocial, and environmental evaluations.
Develop and implement individualized Plans of Care (POC) based on physician orders, patient needs, and clinical judgment.
Complete OASIS documentation accurately and in compliance with Medicare, Medicaid, and agency standards.
Initiate appropriate clinical interventions and coordinate with the interdisciplinary team to ensure high-quality outcomes.
Provide skilled nursing services such as wound care, medication management, and disease education as needed during the SOC visit.
Educate patients and caregivers on home safety, medication regimen, disease processes, diet, and symptom monitoring.
Communicate promptly and effectively with physicians, case managers, and other healthcare professionals to ensure continuity of care.
Monitor patients for changes in condition and ensure appropriate follow-up and documentation.
Serve as a clinical resource and mentor to field staff when questions arise regarding care plans or patient conditions.
Demonstrate excellent time management and prioritization skills to meet deadlines and maintain compliance.

Qualifications:
Current Registered Nurse (RN) license in Massachusetts
Minimum 1 year of clinical experience (home health experience preferred)
Knowledge of OASIS and Medicare regulations (preferred)
Excellent assessment and communication skills
Reliable transportation and valid driver’s license
