Palliative Care & Hospice NP (or PA)

Join an expanding Palliative Care & Hospice team.  Outpatient position.   Monday - Friday, 8a - 5, no weekends, no call.  Support 6 patients per day.   Practice autonomously.  Assess, plan, implement, and evaluate care needs.  Symptom management, chronic disease management, goals of care, advanced care planning.

Requirements:

• BC Family Medicine or Internal Medicine
• Palliative Care & Hospice experience desired
• Geriatric Certification required, or willing to obtain within 1 year
• OH license, or eligible
