Job Description
The primary responsibility of the Strategic Housing Coordinator is to align housing initiatives with healthcare funding (Medicare/Medicaid) streams to support vulnerable populations. This position bridges the gap between healthcare and housing systems, ensuring equitable access to diversion and rapid rehousing services. designing, implementing, and coordinating housing strategies that support individuals experiencing housing instability, particularly those eligible or not eligible for OHP-funded Health-Related Social Needs (HRSN) housing supports.
ESSENTIAL JOB RESPONSIBILITIES
Develop and implement housing strategies that leverage Medicaid authorities (e.g., 1115 waivers, state plan amendments, managed care contracts) to fund housing-related services
Align housing initiatives with state Medicaid goals, such as reducing hospital readmissions, improving health outcomes, and addressing social determinants of health
Identify and coordinate funding streams, including Medicaid Health-Related Services (HRS), Supporting Health for All through REinvestment (SHARE), and Medicare flexibilities
Ensure compliance with CMS guidelines, especially regarding non-coverage of room and board, while maximizing allowable services like tenancy support, case management, and housing navigation
Facilitate partnerships between Medicaid agencies, housing authorities, behavioral health providers, and community-based organizations
Serve as a liaison to Continuums of Care (CoCs), public housing agencies, and managed care organizations to align housing and health services
Oversee the delivery of Medicaid-funded housing support such as:
Housing transition services
Tenancy-sustaining services
Environmental modifications
Behavioral health supports
Monitor service quality and ensure integration with healthcare and social services.
Track housing stability, health outcomes, and cost savings using Medicaid claims data and housing metrics.
Report on program performance to funders and policymakers to support sustainability and scaling
Advocate for policy changes that expand Medicaid’s role in housing, including new waivers or pilot programs.
Stay informed on federal and state innovations, such as the Medicaid Innovation Accelerator Program and Accountable Health Communities model
