The Licensed Dietician will help our community thrive by providing nutrition education with a focus on diabetes patients, to support behavior modification training to assist patients with care plan implementation, and to collaborate with the providers on care plan initiation.
Responsibilities
Serves as a nutrition and diabetes consultant to the healthcare team and community
Assemble information concerning the patient's clinical background and referral needs. Per referral guidelines, provide appropriate clinical information to the provider and/or specialist
Establish and maintain relationships with identified service providers
Ensure that referrals are addressed in a timely manner
Will need to have strong verbal communications and skills for a positive interaction with providers and their patients
Utilize critical thinking for determining alternative solutions and assist in solving patient problems and issues as the need to be in compliance with the diabetic disease
Utilize ADA and AADE approved curriculum and education materials for patient instruction
Utilize appropriate motivational interviewing techniques necessary for coaching and assisting the patient to complete self-management goal/action plans
Collaboratively develops goals, learning objectives, and a plan for educational content and teaching methods
Provide education interventions that utilize interactive, skill-based training methods. Group classes and Individual teaching
Develop individualized follow-up plans with patients
Document assessments, educational services, and results for each patient
Assess program effectiveness utilizing defined outcome measures
Identify when a program participant’s needs are outside the scope of practice and expertise; arranges for additional services to meet needs
Provide education on medical devices which include: Blood glucose monitoring is required, insulin pumps, continuous glucose monitors, and other insulin delivery systems are preferred but not required at time of hire
Qualifications and Education Requirements
Board-certified RD/RDN
1+ years clinical experience. Diabetes education experience preferred
Certified Diabetes Care and Education Specialist (CDCES) preferred or willing to obtain the certificate within 1 year of hire
Must have proven competencies in nutrition education and education activities
The RD must demonstrate the knowledge and skill necessary to complete initial patient/family educational sessions, assessments, reassessments, and follow-up with accuracy and skill.
Must have effective problem-solving ability, accountability, flexibility, and must be a team player
Build effective collegial relationships, influence and effect change
Good time management skills, positive, and service-oriented attitude
High level of integrity and maintain a HIPAA compliant professional work environment
Must maintain valid driver’s license and vehicle
Develop, implement, communicate and evaluate a plan of care for each call
Bi-lingual applicants preferred
