The Social Worker supports individuals and their families through difficult times and ensure that vulnerable people, including children and adults, are safeguarded from harm. Their role is to help in improve outcomes in people's lives.
Role and Responsibilities
Evaluate the psychosocial and emotional needs of members with ongoing and/or situational medical concerns through coaching, education, and support for low acuity emotional needs.
Coordinating referrals for behavioral health services when their emotional/mental health needs are more chronic in nature.
Will assist the member in both inpatient or outpatient settings and will provide discharge planning assistance, as appropriate, during an inpatient stay.
Serves as a liaison with the community resources and their staff to facilitate smooth transition and placement of the patients within the assigned service line.
Advocate for and help clients get resources that would improve their well-being
Research and refer clients to community resources, such as food stamps, child care, and health care
Assess clients’ needs, situations, strengths, and support networks to determine their goals
Develop plans to improve their clients’ well-being
Help clients adjust to changes and challenges in their lives, such as illness, divorce, or unemployment
Help clients work with government agencies to apply for and receive benefits such as Medicare
Respond to crisis situations such as child abuse
Follow up with clients to ensure that their situations have improved
Evaluate services provided to ensure that they are effective
Consult with doctors, therapists and medical professionals
Administer social service programs as determined by the company
Instruct clients’ families during treatment when necessary
Qualifications and Education Requirements
Master’s Degree in Social Work
1 years clinical social work experience
Valid and current LCSW or LMSW license
Ability to make psychosocial assessments and develop and implement viable care plans
Excellent listening and communication skills
Bilingual preferred
Experience with EMRs
Ability to provide after-hours service as needed
CCM or CPHQ (or equivalent) highly desirable
Demonstrated knowledge in the areas of health improvement, process improvement, population health, and health systems planning
Thorough knowledge of medical management clinical review criteria, processes and techniques
Computer Skills: Microsoft Outlook and Word with advanced Excel skills
