Job Summary: We are seeking a compassionate and experienced Substance Use Disorder (SUD) Case Manager to join our team. The ideal candidate must possess either a Certified Drug and Alcohol Counselor Associate (CDCA) license or Peer Recovery Support certification in the state of Ohio and have familiarity with drug court processes, probation services, and experience working within the criminal justice system—particularly with drug court clients—as well as experience collaborating with the Ohio Department of Job and Family Services (ODJFS). The SUD Case Manager will provide client-centered support and coordinate care for individuals involved in the justice system struggling with substance use disorders, facilitating their access to treatment and community resources while promoting successful recovery and reintegration.
Key Responsibilities:
Conduct comprehensive assessments addressing clients’ substance use, legal involvement, mental health, and social needs.
Develop, implement, and monitor individualized care plans aligned with clients’ recovery goals and legal requirements.
Coordinate services including treatment programs, probation requirements, housing, employment, social supports, and connections to ODJFS resources.
Serve as a liaison between clients, drug court personnel, probation officers, treatment providers, ODJFS representatives, and other stakeholders.
Advocate for clients to ensure access to appropriate services and compliance with legal and social service mandates.
Maintain detailed, confidential documentation in accordance with agency policies and regulatory standards.
Provide peer support and education focused on recovery, relapse prevention, and navigating the criminal justice and social services systems.
Collaborate with multidisciplinary teams including healthcare providers, criminal justice representatives, ODJFS staff, and community resources.
Participate in outreach and engagement efforts to support justice-involved clients and those interfacing with state social services.
Monitor client progress toward recovery, legal compliance, and social service engagement, adjusting care plans as needed.
Qualifications:
Demonstrated familiarity with drug court processes, probation services, and experience working within the criminal justice system, especially drug courts.
Experience collaborating with or navigating services through the Ohio Department of Job and Family Services (ODJFS).
Strong knowledge of substance use disorders, recovery models, and community resources.
Excellent communication, interpersonal, and organizational skills.
Experience in case management or peer support within justice-involved populations preferred.
Ability to maintain detailed and confidential client records.
Knowledge of relevant local, state, and federal regulations.
Commitment to culturally competent, client-centered care.
Preferred Skills:
Valid Certified Drug and Alcohol Counselor Associate (CDCA) license OR Peer Recovery Support certification in Ohio.
Crisis intervention and de-escalation techniques.
Motivational interviewing and harm reduction strategies.
Experience with electronic health records (EHR) systems.
Working Conditions: This role involves office-based work as well as outreach in community and justice settings. Flexibility to work evenings or weekends may be required based on client needs and court or agency schedules.
