    About the role:

    As a Nurse Practitioner, you will care for people with cognitive decline and their families. This includes people concerned about cognition and want to establish a baseline for their brain health, those with symptoms seeking a diagnostic evaluation, and those with an established diagnosis seeking tailored care plans.

    • Responsibilities:
      • Performing new patient visits, including acquiring a cognition-focused history, completing cognitive, psychological, behavioral, functional, and caregiver assessments, ordering and interpreting labs and imaging, making and verifying diagnoses, and developing care plans.
      • Providing continuity of care by performing serial whole-person and family-focused assessments and creating and revising care plans to meet their evolving needs as the disease progresses.
      • Administering neuropsychological testing and conducting capacity evaluations.
      • Completing medication review and reconciliation and deprescribing high-risk medications.
      • Providing an integrative approach to the management of the behavioral and psychological symptoms of dementia.
      • Collaborating with primary care providers and specialists to eliminate gaps in care.
      • Empowering people with cognitive decline and their families by providing patient education, anticipatory guidance, and tactical strategies to manage symptoms and changes as the disease progresses.

    Qualifications:

    • Education & Licensure:
      • Master's or Doctorate in Nursing.
      • Licensed Registered Nurse and Nurse Practitioner in the District of Columbia, Maryland, and Virginia. Additional state licenses are nice to have.
      • Certified Adult-Gerontology or Family Nurse Practitioner.
    • Qualities:
      • Emotionally intelligent.

    • Experience (2+ years):
      • Evaluating patients with mild cognitive impairment and Alzheimer's disease and related dementias.
      • Managing the behavioral and psychological symptoms of dementia.
      • Partnering with patients and families to create advanced care plans.
      • Delivering telehealth services.
      • Maintaining a strong evidence-based clinical skill set while practicing and implementing outcome-focused care.
      • Feel comfortable working with or learning various technology systems and tools.
      • Possess an entrepreneurial spirit or have previous experience within a startup or highly dynamic environment.
