    $500.00 SIGN ON BONUSSuperior Home Care & Hospice is a locally owned and operated agency focused on high quality patient care. We've been in business since 1996. As our growth continues, we are seeking a part time Social Worker. We are here to exceed the expectations of our patients, clients, and their families by providing excellent care.

    Duties include:

    Providing psychosocial education to patients and family caregivers about coping skills, hospice and palliative care philosophy, and nonpharmacological symptom management strategies;

    Providing in-services to other service providers and organizations;

    Planning for discharge, coordinating care, and helping clients navigate systems;

    Overseeing the hospice bereavement and volunteer programs;

    Intervening in crisis situations;

    Mediating conflicts within families, between clients and the interdisciplinary team, and between service organizations;

    Participating in interdisciplinary team meetings, care planning, and ethics consultations;

    Advocating on behalf of the patient and family;

    Identifying and linking clients with resources;

    Requirements include:

    Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Educations

    Strong organizational skills

    Attention to detail

    Ability to build rapport with patients and families

    Strong problem solving and critical thinking skills

    Ability to thrive in a fast-paced environment

    Ability to communicate effectively, both verbally and in writing

    Current Utah driver's license and automobile insurance

    CPR certification.

    Hospice and/or home health experience is preferred.

    If you have a passion for providing high quality patient care, have good documentation skills, and enjoy caring for patients in a home setting, then this is the job for you and we look forward to hearing from you. This job is approximately 20-25 hours per week and there is some flexibility with the schedule. You would need to be available for at least 4-5 hours each day Monday - Friday.

    Salary: DOE*Sign on bonus paid after 12 weeks of employment
