Job Title: Supervisor of Clinical Services – RN (Home Health)

We are a compassionate, community-focused home care organization committed to excellence in person-centered health care. We are currently seeking a seasoned and dedicated Registered Nurse to join our leadership team in the role of Clinical Services Supervisor.
Position Overview:
The Supervisor of Clinical Service will oversee the coordination and quality of clinical care provided to clients in their homes. This individual plays a vital leadership role and may serve as the Administrator when needed.
Primary Responsibilities:
Oversee and support the delivery of direct care services by clinical staff
Manage new referrals and ensure proper admission assessments
Monitor and revise individualized care plans based on patient needs
Provide supervision, support, and performance evaluations for nursing staff
Contribute to the development of clinical standards, procedures, and objectives
Participate in hiring, onboarding, and training of clinical personnel
Support agency outreach, marketing efforts, and client development initiatives
Ensure regular communication with clients, families, and staff
Deliver direct patient care and complete required documentation promptly
Maintain compliance with infection control, safety, and regulatory protocols
Monitor documentation timelines and adherence to payor guidelines
Participate in on-call rotations and provide clinical leadership as needed
Supervise no more than 15 FTE clinical staff and evaluate home health aides
Minimum Qualifications:
RN licensure in Connecticut, in good standing
One of the following education/experience paths per state regulations:
MSN + 1 year in home health
BSN + 3 years clinical (1 in home health)
ADN/Diploma + 3 years recent clinical (1 in home health) + applicable community health training
Skills & Competencies:
Strong leadership, communication, and mentoring skills
Knowledge of home health regulations and care standards
Skilled in care coordination, staff oversight, and quality assurance
Comfortable using electronic charting systems and secure communication tools (e.g., EMR, TigerConnect)
