The Support Services Case Manager will assist Brain Injury waiver participants with the development of a waiver plans of service, monitoring waiver services, monitoring the health and welfare of waiver participants, and assisting with the development and implementation of a quality management program. This position will also provide brain injury training and technical assistance to behavioral health providers, law enforcement, caregivers, and other community partners.
The duties and responsibilities listed below are intended to describe the general nature of work to be performed by individuals in this job. Job responsibilities shall be performed under the supervision of the Brain Injury Association Assistant Director.
Duties and Responsibilities
Develop Plans of Service for Traumatic Brain Injury Waiver participants that adequately address the individual’s needs, choices, and strengths.
Assist the Office of Older Adults and Long-Term Services and Supports with the development and implementation of a Quality Management program for the Waiver.
Provide Quarterly site visits to waiver participants to review waiver plans of service, monitor waiver services to ensure that they meet the participants’ needs and that they are cost affective, and ensure the safety and wellbeing of waiver participants.
Provide Community Settings Questionnaire with brain injury waiver participants to determine proper placement within a residential and day habilitation program.
Assist Behavioral Health Administration Office of Older Adults and Long-Term Services and Supports with waiver quality assurance activities.
Attend quarterly Brain Injury Waiver Provider meetings.
Assist Behavioral Health Administration Office of Older Adults and Long-Term Services and Supports with increasing provider capacity by training and recruiting providers.
Utilize the Long-Term Services and Supports Tracking system to maintain all necessary case records as required.
Participate in team meetings for the purpose of monitoring and evaluating the participant's progress.
Provide technical assistance and/ or training to TBI waiver providers.
Enhance professional growth and development through participation in educational programs, seminars, and workshops.
Conduct community surveys and collaborate with other health specialists and civic groups to identify health needs, develop desirable health goals, and determine availability of professional health services.
Develops and maintains cooperation between public, civic, professional, and voluntary agencies
Prepares and disseminates educational and informational materials.
Provide brain injury training and technical assistance to behavioral health providers, law enforcement, caregivers, and other community partners such as schools, nursing facilities, and hospitals.
Plans for and provides educational opportunities for health personnel.
Create or curate webinar training(s) educating the public about brain injury or brain injury specific resources.
Other duties and responsibilities as delegated.
Qualifications
I. EDUCATION: Master's degree in Social Work or social service field preferred, or Bachelor's degree in social service field with a minimum of two years of experience in case management or resource coordination
II. Knowledge and understanding of brain injury, Certified Brain Injury Specialist preferred
III. Relevant work experience with individuals with brain injury
IV. Relevant work experience with curricula development, training, and program evaluations
V. Good communication skills
VI. A valid Maryland Driver's license with no more than two (2) points accumulated on the driving record.
