    Summary

    Under the direction of the Outreach Coordinator, your role is to provide outreach services, information and referral, and assistance and support services for veterans who are older adults and adults with disabilities in the City and County of San Francisco. The programming will educate, empower, and support veteran older adults and adults with disabilities to access needed benefits and participate in services, and will provide services designed to support individuals to live independently in their own homes and communities.

    Key Responsibilities

    ~Must be able to communicate effectively with a variety of personalities and be comfortable embracing the agency’s Diversity, Equity, and Inclusion Initiative and the principles of Cultural Humility.

    · Conduct outreach targeting veteran older adults and adults with disabilities to increase engagement in veteran services provided by the agency and other community organizations.

    · Outreach will be conducted at community events and public places where veterans frequent, including businesses, public spaces, non-profit and/or faith-based organizations, health care centers, housing sites, etc.

    · Provide one-to-one assistance to clients in accessing services including forms/application completion assistance, housing, benefits, or income support, contacting agencies on behalf of the client, completing on-line referrals, writing letters, making phone calls to set up or confirm appointments, escorting the client to service providers, and conferring with service providers to ensure the client’s needs will be met.

    · Adhere to laws regarding confidentiality and reporting requirements (HIPAA).

    · Adhere to STP and various funders policies and procedures when in the field.

    · Timely submission of required documentation.

    · Must be able to perform all essential job requirements and responsibilities within what would be considered reasonable accommodation

    · Performs other related duties as required by the Outreach Coordinator and/or Clinical Director of Non-Residential Services.

    Education

    · Two-year college degree or equivalent combination of education and/or work experience

    Experience

    · Translatable military experience and skills may be alternatively substituted

    · Experience conducting outreach to unhoused individuals

    Required Skills

    · Computer skills with proficiency in Microsoft Office- Outlook, OneDrive, Word, Excel, etc.

    · Valid Driver’s License in good standing- must be able to drive agency van

    · Excellent verbal, written, and communication skills

    · Strong customer service skills

    · Comfortable presenting in front of others

    · Flexibility and willingness to perform multiple jobs in a work environment with changing priorities

    · Ability to work independently

    · Ability to follow through with tasks in a timely manner

    · Ability to work cooperatively and effectively as part of a team

    · Ability to develop and maintain ongoing working relationships with community providers

    · A working understanding of the needs of veterans with an emphasis on homelessness, mental health, socio-economic, vocational and substance use challenges.

    · Must be able to communicate effectively with a variety of personalities and be comfortable working with an at-risk client population, including embracing the agency’s Cultural Humility Initiative

    · Ability to withstand prolonged walking and driving when in the field
