    Job Description

    Seeking to hire a highly motivated Safety Coordinator, detailed oriented individual to join our Safety Team.

    DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

    • Driver Qualification Files Administration: On-Boarding & Terminations
    • Drug & Alcohol Testing Program Administration
    • Citations Administration

    CREATE, MANAGE AND MAINTAIN DRIVER QUALIFICATION FILES:

    Enter Driver Information Worksheet data into NSC DQF and Scan all documents;

    o Driver Application for Employment

    o Certification of Compliance with Driver License Requirements

    o Request for Release of Motor Vehicle Report

    o Record of Road Test

    o Prior 7 Day HOS Data Sheet

    o Request for Prior Employer Safety Performance

    o Request for Prior Employer Drug & Alcohol Testing History

    o Obtain a current MVR for all new hires and thereafter annually

    o Obtain copy of Commercial Driver License

    o Obtain copy of Medical Certificate

    § Verify Qualifications of Examiner via FMCSA’s QME Registry

    § Scan copy of medical exam long form, if available

    § Verify Qualifications of Examiner via FMCSA’s QME Registry

    § Scan copy of medical exam long form, if available

    o Scan TWIC, if required

    o Scan Other Supporting Documents into DQF

    § MRO Verification of negative pre-employment drug & alcohol test results

    § PSP Authorization Form

    § Order PSP Report through FMCSA

    § Receipt for Company Drug & Alcohol Testing Policy

    § Receipt for Drug & Alcohol Educational Materials

    § Social Security Card (employee drivers)

    § LNG No Fueling Company Policy Acknowledgement (HLT drivers)

    § Proof of Safety Training Acknowledgement

    § Proof of Omnitracs Training Acknowledgement

    § Proof of Cargo Security Training Acknowledgement

    § Receipt for Entry Level Training (for drivers with less than one-year experience)

    ON-BOARDING/TERMINATIONS:

    • Complete Driver New Hire Checklist & provide with DQF file to Director of Safety for approval
    • Provide new hire with Safety Shoe Voucher and Reflective Safety Vest
    • Ensure applicants enrolled in random drug & alcohol testing pool
    • Enroll all California based drivers in the Pull Program
    • Enroll all drivers and IC’s into their company Random Drug & Alcohol Testing Pool
    • Forward a copy of all DQF documents to PEO
    • Follow-up on all Prior Employer Requests within 30 days and scan responses into DQF

    PERIODIC DOT UPDATES ADMINISTRATION:

    • Monitor periodic driver DOT update renewals
    • Contact drivers and their supervisors to advise of renewals coming due
    • Obtain copy of CDL prior to expiration or place driver Out of Service
    • Obtain copy of Medical Certificate prior to expiration or place driver Out of Service
    • Obtain copy of TWIC prior to expiration

    ADMINISTRATION OF COMPANY DOT DRUG & ALCOHOL TESTING PROGRAM:

    • Establish a working relationship with NSC, our Third-Party Administrator
    • Ensure all frequently utilized clinics have a supply of all required paper CCF’s
    • Ensure our safety office has a supply of all required paper CCF’s for all companies
    • Establish a working relationship with all frequently utilized heath care clinics
    • Ensure all drivers are enrolled in their respective company Random Testing Pool Ensure terminated drivers are removed from Random Testing Pool
    • Receive random testing selections at least once a quarter for all companies
    • Ensure all selections are communicated to the respective operations manager
    • Send Quest Electronic Authorization Forms to driver supervisor
    • Monitor testing results and ensure all required testing is completed during the quarter

    Ensure all Post-Accident (DOT and Non-DOT) is completed timely and recorded properly

    • Document why if DOT Post-Accident testing isn’t completed within required time frame

    § Drug with 32 hours of accident & alcohol within 2 hours or, if not, within 8 hours

    • Obtain documentation of reason for any Reasonable Suspicion testing
    • Maintain secure files for all company drug and alcohol testing records by quarter by company
    • Obtain accurate annual MIS Reports and Bi-Annual Lab Summaries from CDTA for all companies

    CITATIONS ADMINISTRATION:

    • Obtain and process any citations received by any company drivers and company vehicles

    o Visit local courts when necessary to mitigate any company liability for citations

    o Process for payment any legitimate citations

    o Advise respective Operations Manager and Director of Safety of any at fault drivers

    o File and retain copies of all processed citations
