Team Assistant – Hospice Clinical Support
Location: Phoenix, AZ | Full-Time | In-Office
Are you an organized, detail-oriented multitasker who thrives in fast-paced environments? Do you want to make a real difference behind the scenes in healthcare? United Hospice & Palliative Care of Arizona is seeking a full-time Team Assistant to support our clinical care team.
In this administrative role, you'll be the communication hub and operational backbone for our nurses and Home Health Aides (HHAs), helping ensure seamless patient care delivery every single day.
Position Summary:
As a Team Assistant , you'll play a key role in supporting day-to-day operations for our hospice and palliative care teams. From updating medical records and managing visit schedules to coordinating lab orders and respite stays—you'll help keep everything organized, efficient, and compliant.
Key Responsibilities:
Nursing Support
- Send updated medication lists to facilities and pharmacies
- Submit orders for labs, X-rays, and other diagnostics
- Document new patient admissions and schedule initial visits
- Track and record discharges in real time
HHA Scheduling & Support
- Monitor daily schedules and update missed or changed visits
- Manage last-minute call-offs and post urgent coverage requests
- Communicate with team leads and notify facilities when changes occur
IDG Meeting Coordination
- Prepare printed patient lists and agendas for weekly IDG meetings
- Complete and organize post-meeting documentation in our EMR (Consolo)
Respite Care Coordination
- Schedule and set up respite care stays with facilities and families
What We're Looking For:
- 1+ year of experience in healthcare administration or clinical support
- Strong scheduling, time management, and multitasking skills
- Excellent communication and customer service abilities
- Familiarity with EMR systems (Consolo or similar preferred)
- Ability to stay organized and calm in a dynamic care environment
- Prior experience in hospice, home health, or long-term care is a big plus
