    We are a family-owned home health agency (est. 1996) expanding into telehealth/remote patient monitoring (CPT S9001). We’re seeking a dependable, detail-oriented Program Coordinator to help launch and grow this service.

    This is a full-time, in-office role in San Antonio. Your primary responsibility will be our telehealth program — enrolling patients, monitoring readings, and managing blood pressure monitors. At the same time, you’ll support our home health office operations until telehealth volume increases — offering variety and long-term stability.

    What You’ll Do

    Telehealth (Primary Focus — ~60%)

    Enroll patients and physicians into our monitoring program.

    Educate patients/families; occasionally install blood pressure monitors in patients’ homes.

    Monitor daily vitals in EMR; document outreach and escalate per protocol.

    Manage prior authorizations (Molina, Superior, Texas Medicaid, etc.) and payer requirements.

    Track inventory (deliveries, returns, replacements).

    Produce reports for quality audits and compliance.

    Home Health Office Support (Secondary Focus — ~40%)

    Assist with intake/referrals and onboarding new patients.

    Review consents, orders, and documentation in EMR.

    Help ensure charts are complete for audits, ADRs, and RCD spot check.

    Follow up with physicians’ offices for signed orders.

    Support DON and admin team with office coordination and scheduling.

    Qualifications

    LVN license (Texas) preferred, but strong non-clinical applicants with healthcare/telehealth experience will be considered.

    1–3+ years in home health, primary care, RPM, or care coordination.

    Strong EMR experience required — examples: Axxess, WellSky, Kantime (or similar).

    Knowledge of Texas Medicaid and Medicare processes preferred.

    Excellent communication with patients, families, and providers.

    Must be based in San Antonio.

    Bilingual English/Spanish a plus.
