Description:
The Social Adjustment Services (SAS) Case Manager (CM) is responsible for ensuring a collaborative process for coordinating care, assessing, building service plans, and ensuring an appropriate level of communication for services. The CM is expected to use an individualized service plan delivery approach ensuring that services are designed to address the unique needs and strengths of each client. The CM assists the Program Manager and the Program Coordinator in the development and growth of the program’s philosophy, practice, and policies.
Job functions: The below summary may not cover all duties and the position may require the case manager to have additional functions.
· Responsible for assessing clients in order to engage them in organization programs as well as community programs.
· Maintains appropriate documentation, and adheres to HIPPA regulations and client records.
· Responsible for accurately documenting case notes, and assessments, communicating program guidelines, as well as explaining company guidelines and procedures.
· Determines eligibility for client referrals, resources, and internal and external referrals.
· Ability to identify client barriers to services and/or social needs within the community.
· Meet deadlines for reports, service plans, and appropriate contacts to be made with clients.
· Attend weekly, monthly and quarterly meetings. Identifies social needs and barriers with clients, and assesses for various support systems, community networks, and other support.
· Participate in all required internal and external training.
· Ability to be flexible with programmatic needs and changes.
· Work collaboratively with all relevant stakeholders to assess the needs of clients for SAS services.

Required Qualifications and experience:
· Fluency in English and Spanish
· Some college (2-year degree) preferred; 4-year degree a plus;
· In lieu of college degrees, at least 3 years’ experience in behavioral health/clinical settings; data management, non-profit settings, and/or other administrative duties accepted;
· Professional attitude and appearance;
· Community orientated and team player;
· Solid written and verbal communication skills;
· Should be detail orientated and with excellent organization, multitasking and time management skills;
· Ability to be compassionate and calm under pressure, resourceful and proactive when issues arise;
· Hands-on experience with office equipment;
· Proficiency in Microsoft Office Suite preferred;
· High degree of independent judgment, flexibility, and problem-solving skills;
· Must be eligible for employment in the US;
· Must have excellent written and verbal communication skills in English.
