    The SAS Afghan Placement Assistance Program Coordinator serves as one of two team leads for the Social Adjustment Services (SAS) team, ensuring holistic case management services and the coordination of client crises management. The Coordinator provides Social Adjustment Services for eligible populations, including but not limited to newly arrived Afghans, refugees, Asylees, Special Immigrant Visa holders, VOT and Cuban parolees. Services are focused on governmental and community benefits applications, medical case management, and crisis intervention services. Further, the Coordinator supports SAS APA case managers in their case management activities through training, technical assistance and regular check-ins. The Coordinator assists the SAS Program Manager in quality assurance efforts and report writing.

    Responsibilities:

    • Support and train SAS case managers.
    • Support the Manager in completion of weekly, monthly, semi-annual and annual reports of the department and track program performance measures throughout the year.
    • Ensure continuous quality assurance and compliance with programmatic requirements through regular case file review and review of Client Track database.
    • Determine client eligibility prior to enrolling clients in the SAS program.
    • Lead the coordination of client crisis management in the SAS program.
    • Work closely with the Alliance Wellness Center to ensure clients receive holistic mental health services as needed.
    • Support case managers with rental assistance applications for the department.
    • Participate in regular interdepartmental meetings to coordinate agency services.
    • Support organization and coordination of special health education programs, client support groups and regular agency health fairs.
    • Assure and monitor client case file for Refugee Medical Assistance (RMA) eligibility and quality assurance in collaboration with the RMA team.
    • Support clients through holistic case management, including:
    • Medicaid/county health insurance access
    • SNAP application
    • Counseling related to housing issues, education, and general cultural adjustment
    • Referrals to in-house programs and external social service providers
    • Translation and interpretation as needed to help clients access services
    • Provide case management duties to ensure compliance with all contracts and agreements associated with the specific office
    • Maintain case files on all case management clients in the online Client Track database and internal database Refugee Management Solution (RMS).
    • Develop a plan of action to assist clients with barriers to self-sufficiency
    • Assure compliance with all required reports of the SAS program and ensure reports are submitted in a timely manner
    • Serve as a local representative of the agency, by attending meetings, making presentations, and handling public relations duties as required
    • Participate in weekly staff meetings and conference calls as scheduled
    • Maintain a good working relationship with other service providers and community partners in the area
    • Assist the Program Manager with other duties as directed, including airport pick-up

    Qualifications:

    • Bachelor degree or similar college degree required
    • Experience in mediation, crisis management, counseling and family wellness
    • Experience working with foreign-born populations and/or refugees
    • Knowledge of public benefit programs and social services in the Greater Houston area
    • High degree of independent judgment, flexibility and problem-solving skills
    • Valid driver’s license, automobile liability insurance and knowledge of the city
    • Fluency in English, Pashto, Dari and Farsi with excellent written and verbal communication skills
    • Foreign language fluency in a language spoken by refugee populations
    • Experience working with women’s issues is a plus
    • High degree of independent judgment, flexibility and problem-solving skills.
    • Must have a valid driver’s license, automobile liability insurance and knowledge of the city
    • Some evenings or weekends may be required
