    The Bridges of Appleton is a skilled nursing facility located in Appleton, Wisconsin.  It is the mission of the Bridges of Appleton, to allow residents to function at their highest possible level of social, emotional and physical well-being and to maintain their independence and quality of life while providing them with a safe, secure home-like setting.  Furthermore, it is the mission of the Bridges of Appleton,  to provide services in a high quality, efficient and ethical manner in partnership with hospitals, physicians, employees and community organizations. The Bridges of Appleton is currently seeking a RN/LPN PM shift, $3500 sign on bonus, with shift differential.

    The right - RN/LPN candidate for this role would be responsible for the following :

    Orders, interprets, and evaluates diagnostic tests to identify and assess resident's condition

    Monitors all aspects of resident care, including diet and physical activity, and modifies resident treatment plans as indicated by residents' responses and conditions

    Observes resident, records significant conditions and reactions, and notifies supervisor or physician of resident's condition and reaction to drugs, treatments, and significant incidents

     Consults and coordinates with health care team members to assess, plan, implement and evaluate resident care plans

    Maintains accurate, detailed reports and records of residents' medical information and vital signs

    Initiates a resident education plan according to the individualized needs of the resident, as prescribed by physician and/or facility policy including resident and family instruction

    Instructs residents, families and other groups on topics such as health education, disease prevention and health improvement programs

    Responds to life saving situations based upon nursing standards, policies, procedures, and protocol

    REQUIREMENTS

    • Graduate from an accredited nursing program; must be licensed in the state of Wisconsin as a Registered Nurse (RN) 
    • Minimum of 1 year of previous long-term care experience
    • Ability to use desktop/notebook computers, Microsoft Office Suite, various medical equipment (wheelchairs, geri chairs, gait belts, etc.)
