    Description

    The Bridges of Milwaukee,  a skilled nursing facility located in Milwaukee, Wisconsin, is seeking an Activities Aide to assist Health Care Professionals in providing treatment to residents to improve physical, mental and emotional well being.

    Activities Aides implement sports, music hobbies, crafts, dance or other activities based on resident’s capabilities and desires. Generally activities are enjoyable and are modified versions improving some aspect of the resident’s physical or mental well being. Most involve physical effort of some kind, but some may be geared more toward memory or cognition or mental healing.

     DUTIES

    • Speaks with residents and their family or support network and identifies interests.
    • Assists in the planning and engagement of those activities and helping the resident with those activities requiring assistance. 
    • Assist Health Care Professionals in monitoring the success of the chosen activities and assists residents and Lead HCP set reasonable and obtainable goals.
    • Facilitates activities and events that help residents maintain physical and mental alertness that will allow them greater enjoyment of life.
    • Modifies the activity to make it easier physically or cognitively to maximize the resident’s full level of independence and engagement.
    • Challenges residents to stretch higher, move faster, or make broader movements and keeps them motivated while having fun.
    • Introduces recreational therapy in groups or a one on one setting.
    • Completes ADL and Activity documentation, and prepares notes and periodic reports reflecting Residents' reactions and evidence of progress or regression.
    • Maintains working knowledge of the State Regulations & survey process for Long Term Care as it applies to the position.
    • Maintains basic knowledge of the human body and its associated mechanics of movement, human growth and development.
    • Observes and reports resident behavior as it relates to residents’ participation in their prescribed care.
    • Fosters positive relationships with residents and their family or support network.
    • Reports any issues or problems that may arise to the Activity Director.
    • Complies with state, federal, and all other applicable health care and safety standards.

    Performs other duties as directed.

     Requirements

    • At least 1 year of Long Term Care Activity Aide experience preferred
    • Certified Nursing Assistant Certificate (Wisconsin) preferred
    • Familiar with Microsoft Office Suite preferred
    • PCC knowledge or other Long Term software
