    ESSENTIAL FUNCTIONS: Provides assistance to residents with ADL’S and IADL’S and assists in the maintenance of the facility environment. Participates actively in the provision of care services to the resident.

    OPENINGS:

    The Cottages at The Glen - The Cottages provides a comfortable, cozy assisted living environment for those living with memory loss. In this unique setting, our residents can enjoy his or her own individual apartment, while still receiving some assistance with bathing, dressing, and other support services.

    Shifts: 3:00 p.m. – 11:00 p.m. OR 11:00 p.m. – 7:00 a.m. (4 on 2 off)

    ADDITIONAL FUNCTIONS:

    Receives and gives report to charge nurse & team members at change of shift.

    Encourages or assists with activities of daily living(ADLs) such as bathing, grooming and with instrumental activities of daily living(IADL) such as shopping, housekeeping, laundry, socialization

    Practices effective communication techniques with residents, visitors, and coworkers and promotes a positive image of the facility

    Arranges, schedules, encourages resident activities, both spontaneous and scheduled

    Adheres to infection prevention and control practices

    Assists with admissions, transfers, and discharges of residents

    Prepares apartments for new admissions

    Transports and escorts residents

    Mentors new employees

    Documents care as necessary

    Participates in development, implementation, and evaluation of residents’ service plan

    Keeps environment, equipment, supplies, and resident furniture clean.

    Reports to supervisor:

    Changes in residents’ status

    Equipment and supply needs

    Repair and maintenance needs

    Safety hazards

    Unusual occurrences or observations

    Problems with completions of assignment

    Directs resident and visitor concerns to supervisor

    Responsible for obtaining required in-services

    Adheres to posted schedule. Submits requests for time-off per facility policy

    Responsible for adhering to all safety guidelines for protection of self, residents, and co-workers

    Works to promote maintenance of residents’ abilities at their highest level.

    Protects residents’ privacy

    Responds to emergency calls from residents

    Other related projects, duties and responsibilities may be assigned as deemed necessary.

    QUALIFICATIONS/REQUIREMENTS:

    Certification as a Nursing Assistant with the State of Louisiana preferable.

    High school graduation or equivalent.

    Able to read, write, and follow oral and written instructions

    Basic computer skills to document with EHR

    Able to attend regularly scheduled in-services provided by the facility

    Visual and hearing acuity to assess and provide resident care.

    Must be people-oriented , have good communication skills, and the ability to work with a diverse group of people

    PHYSICAL DEMANDS:

    Must be able to fulfill job requirements in a high-energy, high-traffic area.

    Must be able to lift up to 50 pounds; push, pull up to 90 pounds.

    Finger and hand dexterity to operate small equipment and perform job tasks

    Extensive sitting, standing and walking
