    The Glen Retirement System is seeking an experienced, dedicated and

    organized PRN RN with excellent nursing and administrative skills for

    our 126-bed Long-Term Care facility, Village Health Care.

    SCHEDULE VARIATION: Normally scheduled to work on an “as needed” or casual basis. (Weekend and after hours work apply)

    ASSIGNED SHIFT: Shift is normally an 8 or 12 hour shift, but could be more or less and hours could be changed depending on the needs of The Glen. (Assignment may vary)

    ESSENTIAL FUNCTIONS: In physical absence of the Director of Nursing, responsible for insuring provision of quality resident care in accordance with facility standards and supervising nursing department while on duty. Completion of Administrative Nursing Tasks as Assigned. Provides direct resident care as assigned or as needed.

    Administrative

    Is responsible for management of nursing department as assigned in the physical absence of the DON. Directing and monitoring to insure quality resident care in compliance with laws and regulations and in keeping with the standards of The Glen and The Glen’s Mission Statement.

    Has good rapport and communication with Nursing Director, reporting all risk management issues and significant changes with residents’ conditions in a timely manner.

    Is liaison with Director of Nursing, resident’s family members, and physicians.

    Coordinates clinical care with ancillary departments and services.

    Regular rounds on all residents in facility with specific focus on acute medical or behavior problems, condition changes, and risk management issues.

    Communicates resident and family grievances to the Administrator and Director of Nursing. Participates in grievance investigations and resolutions.

    Participates in activities related to facility inspection or investigation by regulatory or accrediting agencies.

    Monitors to assure residents’ rights are protected and residents are free from abuse and neglect

    Participates as assigned in the facility Quality Assurance and Performance Improvement program.

    May be assigned to assist with information pertaining to the Facility Assessment.

    Assist with the facility’s infection control program as assigned.

    Serve as the Weekend RN supervisor as assigned.

    Adheres to a professional code of ethics.

    Education

    Participate in the orientation and training of nursing department staff as assigned.

    May in-service staff on outcomes of quality data, infection trending, and interventions for Resident care needs.

    Conduct skills competency evaluations and training as assigned.

    Maintains and improves competencies by participating in and attending professional educational programming.

    Clinical Mentor

    Communicates with Household nurses. Offers advice, guidance, and support to address clinical issues and resident or family concerns. Serves as a resource to staff in clinical and managerial decision-making.

    Performs resident assessments and make updates to the resident plan of care as assigned.

    Participates in development of the resident plan of care as assigned

    Responsible for reviewing incident reports as assigned. Participates in investigation into root cause. Collects data, provides guidance and resources to implement preventive measures and documents feedback.

    Communicates residents’ condition and changes in status to physicians. Rounds with physician as needed.

    Provides guidance to nursing staff on special care needs including but not limited to IV therapy, wound care, chronic disease care management and monitoring.

    Ensures lab, radiology, and other diagnostic service reports are acted on per facility policy.

    May be assigned to monitor medication rooms, in-room medication storage, medication carts, and emergency pharmacy supply kit for regulatory compliance with state, federal, and pharmaceutical agencies.

    Receives and transcribes physician orders.

    Perform wound care rounds as assigned. Makes assessments and recommendations regarding treatments and preventive measures. Documents measurement, staging, and charts healing progress.

    May be assigned to answer consultant reviews. Communicates trends and takes corrective action. Provides report to the Director of Nursing and Medical Director.

    May be assigned to monitor medication and treatment passes with Household nurses. Provide guidance on regulatory and infection control compliance. Has input on competency.

    May be assigned to assist Medicare nurse and MDS coordinator with completion of MDS.

    Assists with clinical emergencies.

    May provide direct resident care or serve as nurse leader / charge nurse of a household as needed. Administers medications and treatments per physician’s orders. Documents in the clinical record.

    Other duties as assigned by Director of Nursing or Administrator.

    Other related projects, duties, and responsibilities may be assigned as deemed necessary.

    SUPERVISORY RESPONSIBILITIES:

    Supervises staff of nursing department as assigned in the absence of the DON

    Supervises Household nursing staff when serving in the role of household /charge nurse.

    QUALIFICATIONS/REQUIREMENTS:

    Registered Nurse – Current Louisiana license

    Long Term Care experience preferred.

    Possess communication, time management, and organizational skills.

    Clinical nursing skills.

    Management and leadership skills

    Must be people-oriented individual, have good communication skills, and the ability to work with a diverse group of people.

    Must be able to work overtime as required.

    Computer skills appropriate to job functions

    Visual & hearing acuity to assess residents health status

    Finger/hand dexterity to operate small equipment

    PHYSICAL DEMANDS:

    Must be able to fulfill job requirements in a high-energy, high-traffic area.

    Must be able to lift up to 50 pounds; push/pull up to 90 pounds.

    Extensive bending, stooping, walking, standing
