    SUMMARY

    A person is this position is responsible for day-to-day programming tasks associated with the Community Participation Supports (CPS) and/or Home & Community Based Services (HCBS) program as well as the care of the individuals attending the program. Work assignments are varied in nature and are performed under the supervision of the CPS Manager or Specialist.

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    Ensures that each individual participates in social/leisure/recreational activities based on individualized plans. This includes planning events/activities, assisting with scheduling, arranging transportation, taking attendance and various other program related tasks.

    Completes functional tasks for programming such as meeting minutes, shopping, vehicle logs, vehicle maintenance and petty cash counts.

    Provides support to the participants in activities of daily living such as personal/oral hygiene and appropriate dress; requires the transfer of non-ambulatory participants using mechanical apparatus or approved lifting/transfer techniques.

    Performs chore duties and assists/supervises the CPS attendees in performing these duties to ensure that the day program facility is clean, safe, and secure at all times. Duties include such tasks as vacuuming; sweeping; dusting; doing dishes; stocking supplies; cleaning appliances, countertops and tables.

    Assists in the training of new staff to ensure continuity of training.

    Assists to ensure adequate staff in place at all times; performs direct care as required.

    Meets directly with Program Specialist/Manager to coordinate services.

    Assists with coordinating and scheduling specialized services for individuals.

    Prepares and completes required documentation in compliance with state regulations, company standards and the policy and procedure manual; assists assigned CPS staff in completion of daily and monthly documentation.

    Reports questions and/or concerns in regard to programmatic or individualized issues to the Program Specialist.

    Attends required training sessions and meetings as required.

    Dispenses medication prescribed for the clients by strictly following the medication administration procedures for administering and charting of medicines.

    Openly communicates with other service providers and caregivers for the well-being of the participants.

    PHYSICAL DEMANDS

    The work environment for this position varies between a facility setting and community based locations.     TRAVEL

    This position requires a minimal amount of personal travel; however, extensive travel utilizing company-owned vehicles is required on a frequent basis.

    REQUIRED EDUCATION AND/OR EXPERIENCE

    Must be at least 18 years of age and have a valid Pennsylvania driver’s license with good driving record. A high school graduate or possession of a general equivalency diploma is preferred.

    PREFERRED EDUCATION AND EXPERIENCE

    Preference given to level 2 or 3 staff (RDS only)

    2 years of experience in CPS services

    QUALIFICATIONS

    Language Skills

    Ability to read and understand documents and instructions in English. Ability to respond effectively to inquiries.

    Mathematical Skills

    Basic mathematic ability including addition, subtraction, multiplication, and division.

    Reasoning Ability

    Ability to read instructional bulletins and/or participate in training sessions then translate information provided into day-to-day work duties.

    ADDITIONAL ELIGIBILITY QUALIFICATIONS

    Ability to develop and maintain a positive and understanding attitude toward day program participants.

    Ability to make sound judgments regarding the health and welfare of day program participants.

    Ability to respond to emergency situations appropriately.

    Ability to work as a member of a team cooperatively and ethically to ensure the day program is a safe, happy, and clean environment.

    Ability to express oneself clearly, orally and in writing, and to follow instruction and direction in the performing of job tasks.

    Ability to establish effective working relationships with co-workers, supervisors, families, and others who are essential for the well-being of day program participants.

    Ability to learn and comprehend behavior management and program planning principles and applicable safety and health measures.

    Knowledge of the principles and methods of rehabilitation for program participants.

    Knowledge of health measures such as basic first aid, CPR, fire safety and personal safety measures.

    Knowledge of basic principles and methods of staff supervision.

    Ability to plan, organize, train, assign, monitor, and evaluate the work of staff.

    Ability to analyze situations and formulate an effective course of action.

    Ability to secure the confidence of and develop and maintain an empathetic understanding of clients.

    Ability to organize and prioritize various work assignments.

    Ability to develop and implement individual goal plans and program activities for program participants.

    Ability to establish and maintain effective working relationships with county and state officials, clients and their families, and the general public.
