    The ReDCo Group is seeking an innovative and purpose driven individual to join our team as a Case Manager - Family Support Unit. The work that is performed is done in a team approach and involves initiative and independent professional judgment.

    RESPONSIBILITIES:

    Completes family assessments to determine eligibility to receive home-based services and, through the therapeutic milieu provided in the home, encourages the unity and empowerment of families and the development of individual capacities• Coordinates services within the mental health system and between all agencies involved with each assigned facility• Provides linkage into needed mental health and other agency services in addition to or as follow up to family-based services• Provides relief services to assigned families if prescribed in the family service plan or links family to alternate provider• Documents patient records in accordance with state and local regulations and program policies and procedures• Presents case information and recommendations for each case at interagency team meetings• Participates in the family-based on-call crisis intervention component• Takes part in the gathering of information to demonstrate client progress• Attends and completes all components of Family based training model• Performs related work as required• Transports clients to appointments

    QUALIFICATIONS:

    Completion of a Bachelor’s Degree in Psychology or related field and one year of experience• Possession of a valid Class C Pennsylvania driver’s license with a clear driving record
