    SUMMARY:

    A person in this position functions under the working title of TAY Psych Rehab Worker. Assignments are varied in nature and are performed under the direct supervision of the TAY Psych Rehab Director.

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    Utilizes recovery and resiliency principles and approach to create a welcoming environment for participants• Teaches rehabilitation and independent living skills to participants• Assist participants in development of individual service plans and provides support to participant in the completion of these goals• Schedules and runs group and individual activities in support of recovery plans and independent living skill development such as budgeting• Participates in weekly supervision and clinical review of current participant cases• Provides transportation for participants when appropriate and within the scope of their individual service plans• Provides Case management services for participants such as referrals, advocacy, and crisis intervention as required• Completes all required documents in accordance with all policies and procedures and regulatory guidelines• Participates and organizing Advisory Council meetings for participants• Provides on-call services to SILP program on a scheduled basis, including contacting local contractors and vendors to assess and manage property related emergencies• Completes required trainings yearly in order to obtain CPRP certification• Maintains a minimum productivity expectation of 70%

    REQUIRED EDUCATION AND/OR EXPERIENCE:

    A bachelor’s degree.• An associate’s degree and 1 year work experience in mental health direct service. • A CPS certificate and 1 additional year paid or volunteer work experience in mental health direct service.• A high school diploma or GED and 2 years work experience in human services which must include 1 year of mental health direct service.

    PREFERRED EDUCATION AND EXPERIENCE:

    Experience working with directly with Transition Aged Youth mental health consumers, providing services involving case management, individual or group therapy, crisis intervention, vocational training, residential care or social rehabilitation in a mental health facility or program preferred• Experience working in a Psych Rehab program preferred

    QUALIFICATIONS:

    Language SkillsAbility to read, analyze, and interpret documents. Ability to implement plans established for residents and related to individuals in a positive and empathetic manner. Ability to supervise residents and respect resident rights. Ability to read, understand, and abide by all policies and procedures for agency and program.

    Mathematical SkillsAbility to apply basic mathematical concepts such as simple addition and time keeping. Ability to calculate billing units based on start and end time of session

    Reasoning AbilityAbility to define problems, and perform required job duties during emergency situations. Ability to evaluate a crisis situation and determine most appropriate form of intervention i.e. mobile crisis or 911 services.
