    SUMMARY:

    The Site Supervisor is responsible for the direct care of the residents. The Site Supervisor will implement the teaching of daily living skills through the use of goal plans for each resident. This individual will function as a team leader and will be responsible for the supervision and provision of direct care to residents of the ICF/MR and the personal daily living needs of specific ICF/MR residents. Work is performed according to specific ICF/MR guidelines and is carried out under the supervision of the Division Director.

    RESPONSIBILITES:

    Supervises social and recreational activities provided for the residents.• Assists with orientation and ongoing training of Resident Care Advisors under the direction of the QMRP.• Acts as a role model for personnel in regard to interaction with residents, families, and community agencies.• Acts as liaison between direct care staff and facility administration.• Supervises direct care staff.• Attends staff meetings to review facility operation procedures and policy implementations.• Assists staff with proper implementation of resident goals and behavior plan to ensure residents are maximizing individual potential.• Provides supervision and guidance for the personal care needs of the residents of the facility.• Provides transportation to day programming and to social activities as needed.• Ensures conformity to the regulations governing the residential unit so that the unit is clean, safe, and secure at all times.• Assists the residents in learning new skills in the facility, which are appropriate to their strengths, needs, and functional level.• Prepares progress notes and other reports as required by the resident’s individual training plans. These reports are reviewed and approved by the QMRP.• Ensures that the direct care staff report medical symptoms to the appropriate medical personnel.• Communicates progress of the residents in their programming to the Division Director and to the residents family.• Provides direction and counseling so that facility personnel can deal more effectively with job responsibilities.• Participates in the development and implementation of IHP’s designed to improve the social and self-help skills of the residents.• Responds appropriately to emergency situations and follows through with proper documentation.• Provides on call services to facility on a scheduled basis via the QMRP.• Ensures that the residents have adequate supplies of clothing and personal items.• Communicates with other staff concerning the resident’s care orally and through the communication log.• Serves on various committees if elected or appointed.• Attends staff meetings as required.• This position includes supervising all third shift staff.• The management person assigned to third shift and a rotation of all 3 shifts, as deemed appropriate by the facility director, is responsible for supervising direct care staff to ensure that policies and procedures are followed and also that required documentation is completed.• Prepares monthly reports for all consumers including cash on hand, monthly goal reports, and behavioral data tracking reports among various others.• Prepares and facilitates quarterly IHP meetings, behavioral reviews, and monthly staff meetings.• Interviews and trains newly hired staff members.• Actively manage not only staffing needs/concerns, but also manage all facility needs to ensure compliance with ICF regulations and Department of Health expectations.• Performs other related duties as deemed necessary by the Division Director.

    REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:

    Knowledge of the accepted policies and procedures of the ICF/MR Program.• Ability to implement goal plans and guidelines in an effective and organized manner.• Ability to demonstrate the skills and techniques necessary to implement the individual habitation program plans for each resident.• Demonstrates skills and techniques necessary to manage the inappropriate behaviors of residents, and the ability to teach these techniques to direct care staff.• Ability to teach skills to staff and to residents.• Relates in a positive manner to ICF/MR residents.• Ability to supervise residents effectively.• Ability to respect resident rights and to relate to them in an empathetic manner.• Ability to maintain composure in an emergency situation and to guide staff in appropriate actions.• Demonstrates the ability to communicate effectively and appropriately within the established lines of authority.• Ability to communicate effectively orally and in writing with staff, supervisors, families, consultants and the public.

    QUALIFICATIONS:

    Residential Program Supervisors must be at least 18 years of age and possess a high school diploma or equivalent. Additional education and five years’ experience in the human services field is preferred.• Individuals selected for this position must be responsible, trustworthy, and possess the ability to relate to both residents and staff in a positive and cooperative manner. This position must be able to function as part of a team and give guidance and direction in a positive manner.

    **This position involves scheduling hours in order to be available to supervise all three shifts as deemed appropriate by the Division Director. Hours must be scheduled as needed to meet the residents’ needs and for in-service training and meetings.
