    SUMMARY

    A person in this class functions under the working title Family Support Case Manager I and must have successfully participated in the New Hire Stage, maintains a caseload of at least 13 families, submits paperwork within program timeframes and maintains a minimum productivity standard of 75%. This positions functions under the direct supervision of the Director of The Family Support Unit. The work that is performed is a team approach and involves direct knowledge of family and child developmental milestones. We offer a flexible work schedule for this position.

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    Completes risk and safety assessments, progress notes, service plans, quarterly reports, discharge reports and all other program required documentation clearly, accurately, and within required timeframes

    Provides clinical, behavioral, psycho educational treatment modalities based on the assessment of motivational and psychosocial issues, which may include family and group therapies, supervised visitation that focuses on the empowerment of families and the development of skills to utilize during familial interactions.

    Provides court testimony in regard to observed and documented family progress, barriers to service delivery and goal attainment

    Trains and facilitates parents in the strengths and barriers of interactions within the family system to promote a strength-based approach

    Participates in staff development and training sessions as well as regular supervision

    Represents the agency and program in a positive manner at all times

    Abides by all program policies and procedures.

    Transports clients and families as needed ensuring vehicle safety rules are maintained at all times

    Maintains a minimum billable productivity expectation of 75%

    Performs other duties as assigned

    SUPERVISORY RESPONSIBILITIES

    This position is responsible for supervising client activity and ensuring that any supportive visitation services provided are supervised per the guidelines provided through Children and Youth services to ensure the safety and wellbeing of the children involved.

    TRAVEL

    This position requires daily travel and the majority of work time is spent in the community or client homes.

    REQUIRED EDUCATION AND/OR EXPERIENCE

    Bachelor’s degree

    Associates degree in related field with at least 1 year experience working with children or families.

    CNA or LPN with at least 2 years’ experience working with children and families.

    PREFERRED EDUCATION AND EXPERIENCE

    Experience working with directly with children and families, providing services involving case management, knowledge of child development, motivational interviewing techniques, and family systems preferred

    CERTIFICATES, LICENSES, REGISTRATIONS

    Must have valid PA driver’s license

    QUALIFICATIONS

    Language Skills

    Ability to read, analyze, and interpret documents. Ability to implement plans established for families and relate to individuals in a positive and empathetic manner. Ability to supervise visits and respect parent rights. Ability to read, understand, and abide by all policies and procedures for agency and program.

    Mathematical Skills

    Ability to apply basic mathematical concepts such as simple addition and time keeping. Ability to read medication logs and medication labels to ensure resident adherence to prescribed medications.

    Reasoning Ability

    Ability to define problems, and perform required job duties during emergency situations. Ability to evaluate a crisis or abusive/neglectful situation and determine most appropriate form of intervention i.e. mobile crisis, 911, Childline report
