    JOB TITLE: Lead Nurse

    FLSA STATUS: Full-time, non-exempt

    HOURS: All Shifts

    RESPONSIBLE TO: Director of Resident Care Services/ Wellness Director

    Minimum Eligibility Requirements

    • Current state LVN License in good standing

    • Previous experience working with the elderly in a healthcare setting

    • Previous supervisory experience preferred

    • Ability to establish effective relationships with residents, family members, and staff

    • Strong documentation skills

    • Strong organizational skills and ability to re-prioritize daily tasks to accommodate fluctuating needs of the residents and the Community

    • Ability to represent the Community in a positive and professional manner

    Essential Functions:

    Management

    • Assists in developing and monitoring work schedules for all Caregivers and Nursing staff to ensure adequate staffing for quality resident care
    • Assists with hiring process of Caregivers and Nursing staff as directed by Wellness Director and DRCS
    • Assists with marketing responsibilities and networks with referral sources within the healthcare community
    • Assists with training of staff members on health-related topics
    • Acts as a role model to other staff members in terms of professional appearance, communication, and conduct.

    Resident Care

    • Assists with the resident assessment process as necessary
    • Work closely with Wellness Director to ensure resident service plans accurately address resident needs and are updated as resident needs change
    • Assists in monitoring that the medication cart and medication room are orderly
    • Monitors the daily care of all residents
    • Performs skilled nursing tasks as defined by company policy and procedure, and as allowed by state law
    • Responds to all resident emergencies, determining need for and contacting emergency services, and/or physician and family, as appropriate
    • Documents observations, concerns, changes in resident conditions, incidents and /or accidents, and ensures that follow-up documentation occurs until issue is resolved.
    • Reports observations, concerns, and changes in resident condition to Wellness Director, DRCS or Executive Director
    • Communicates resident needs and concerns to family members and physicians
    • Answers and responds to calls from family members, physicians, and other outside healthcare providers
    • Records and implements physician orders, following appropriate procedures for fax and telephone orders
    • Ensures that information and interventions recorded on resident service plans are implemented by Caregivers
    • Obtains reports from caregivers before end of shift
    • Gives report to oncoming Nurse
    • Monitors performance of caregivers for compliance with established policies and procedures, and to ensure quality resident care
    • Reports Caregiver performance issues to Wellnesse Director, DRCS or Executive Director and provides input for Caregiver performance evaluations
    • Manages shift coverage when staff members call in sick for the oncoming shift
    • Acts as Manager on Duty for assigned shift in absence of Administrator or Director
    • Deals appropriately with family member and visitor concerns or complaints, documenting the issues and actions taken through appropriate reporting mechanism
    • Manages minor physical plant issues that arise during assigned shift, reporting as necessary to Maintenance Director.
    • Locks outside doors (p.m. shift); unlocks outside doors (a.m. shift)

    Supervisory Responsibilities of Caregivers

    • Ensures that assigned duties are completed in a satisfactory manner

    Working Conditions:

    • Able to work either full-time or part-time, as agreed upon with Administrator, with some weekend work
    • Available to take some weekend call, as necessary

    ** Physical/Sensory Requirements:**

    Medium work – Ability to exert 20-50 pounds of force occasionally, and/or 10-25 pounds of force frequently, and /or up to 10 pounds of force constantly to lift, carry, push, pull, or otherwise move objects.
