Job Description

We are looking for a dynamic Social Worker (Discharge Planner) to join our social services team in this newly built short term rehab center.  The successful candidate will be proactive, able to solve problems, and address all guest (residents and their family’s) concerns as they transition home. 

Qualifications

WHAT WE NEED FROM YOU:

• Bachelor’s Degree (or higher) in Social Work; LSW is preferred but not required
• Extensive experience with high volume discharge planning
• Has experience in long-term care, skilled nursing and/or rehab center
• Point Click Care (PCC) experience

THIS IS NOT A SUITABLE POSITION FOR A NEW GRAD DUE TO THE FAST PACE NATURE OF THE CENTER.  You need to be able to hit the ground running and must have prior skilled nursing and discharge planning experience.
