Job Description

You will be one of two social services team members for this center.  We are looking for a dynamic Medical Social Worker (Discharge Planner) to join our social services team in this newly built short term rehab center.  The successful candidate will be proactive, able to solve problems, and address all guest (residents and their family’s) concerns as they transition home.

Qualifications

Medical Social Worker (Discharge Planner)  

WHAT WE NEED FROM YOU:

• Bachelor’s Degree (or higher) in Social Work; LSW is preferred but not required
• Extensive experience with high volume discharge planning
• Prior experience in long-term care, skilled nursing and/or rehab center
• Point Click Care (PCC) experience
