Hello! We are TimeDoc Health and are one of the nation's largest providers of care coordination services to medical practices and community health centers. Since 2015, our teams have had significant growth and that growth continues. We are looking to add an OUTREACH COORDINATOR to our growing team of TimeDoctors, and YOU could be our next great addition. We help our clients deliver truly continuous, coordinated care by enrolling their chronically-ill Medicare patients into CMS's Chronic Care Management (CCM) program and providing them with monthly care coordination services. In 2015 we started with 0 patients and 0 medical practices. As a team of TimeDoctors from all over the United States, we have virtually connected to build a department that is engaging and offers a platform for new ideas and continued growth.  Our success so far has been established because we are quick-thinkers that are passionate and supportive about helping to improve the quality of life for those who are chronically ill.  We value being resourceful and problem solvers, give us a challenge and it's game on! In between calls we like to randomly test our wits with some trivia questions and like to engage in some friendly competitions to earn some major bragging rights!  Some of our best ideas that have improved our work-flow have been discussed during our weekly virtual coffee break and we have been a part of many enhancements to our software, company structure, and even company policies.
