Geriatric Case Manager needed in Naples, FL

Salary and Benefits

• The pay range for this position is $55,000
• Health Insurance – 100% covered by employer

Responsibilities

• Monday – Friday (9:00am – 5:00pm)
• Working with the older adult population and families through long-term or short-term case management with the goal of supporting successful independence in the community
• Develop individualized care plans for clients
• Home visits and ongoing contact with clients on your caseload

Requirements

• Master’s or bachelor’s in social work
• Case Management experience or related work
• Excellent verbal and written skills
