Our client is looking for a Hospice Case Manager:

• Salary: $70,000 to $75,000
• Health Insurance: Three Options to Choose From
• Dental: Base Plan: $24.12 Per Month
• Vision: Top Tier Plan: $5.74 Per Month
• Insurance Starts After 30 Days of Employment
• Accrued Paid Time Off: 4 Weeks After One Year
• Retirement Plan: 401(k) with 4% Employer Match
• Tuition Reimbursement: $1,000 Per Year
• Paid Life Insurance

Responsibilities:

• Monday to Friday 8:30AM to 5:00PM
• Backup On-Call Only
• 3-5 Patient Per Day on Average
• Admissions
• Assessments of Patients
• Pain Management
• Coordinate with Physician when needed.
• Anticipate changes and educate the patient, family, and caregivers on how to aid and manage symptoms safely.
• Completion of Annual Skill Checklist

Requirements:

• Licensed Registered Nurse in the State of Pennsylvania
• Coverage Area: North Pittsburgh. Wexford area.
• Preferred Experience: Hospice, Home Health, ER, MedSurge, Oncology
• Excellent Observation, Verbal, and Written Skills
• Reliable Transportation and Active Driver’s License
