Salary and Benefits
- The pay range for this position is $56,000 - $71,000
- Shift Differential: $2.75 per hour every day from 7:00 PM to 7:00 AM

Responsibilities
- Weekends Only: Friday / Saturday / Sunday
- 9:00 AM - 9:30 PM available
- Service Area: Palm Beach County available
- Service Area: Broward County / Boca Raton available
- 7:00 PM – 7:30 AM available
- Service Area: Boca Raton / Boynton Beach / West Palm Beach available
- Carries primary responsibility for evaluation of all patients referred for hospice services.
- Is able to articulate all components of the organization's continuum of care and refer patients and families to those as necessary.
- Assists referring physicians and/or his/her designated liaison with coordinating a plan of care/treatment for hospice appropriate patients and their families.
- Serves as the liaison between our company and physicians, hospitals, long-term care facilities, and other health care professionals.
- Actively markets, through education, the hospice concept to all health care professionals and providers, as well as the community at large.
Requirements
- Clear and active RN license in the state of Florida.
- Adequate and reliable means of transportation with adequate insurance and a valid Florida driver’s license.
- One year in medical/surgical/telemetry or acute care nursing required.
- Hospice nursing experience preferred.
- Bilingual (English/Spanish) preferred.
