Hospice Case Social Worker needed in Anne Arundel, Maryland.

You will join and Hospice company that has been providing Hospice care in Maryland for 36 years.

Salary and Benefits

• $70,000 to $75,000
• Sign on Bonus $5,000
• ($2,500 Paid Out on Day 90) & ($2,500 Paid Out at One Year)

Responsibilities

• Monday – Friday 8 AM to 4:30 PM
• No Call Coverage
• Coverage Area / South Side of Anne Arundel County
• Worker With Patient and Families from Admission to Bereavement Care
• Develop Plan with Patients
• Follow Social Worker Standards of Practice
• Meet or Exceed HOC Quality Goals
• Must be Compassionate

Requirements

• Clear and active license in the state of Maryland
• Minimum of LGSW / LCSW Preferred
• Hospice, Home Health, ICU, CCU, Oncology, End-of-Life Care, or OB experience preferred.
