Palliative Care Nurse Practitioner needed in Baldwin, WI

Titan Placement Group invites you to explore an opportunity in the park-filled area of Wisconsin. The qualified candidate will join a nonprofit community-based hospice and palliative care organization committed to patient satisfaction, clinical excellence, and quality patient care. 

Salary and Benefits

• The pay range for this position is $90,000 to $126,000
• Health Insurance – 80% covered by employer, $55 employee contribution biweekly
• Dental and Vision-100% covered by the employer
• Paid Time Off (10 Vacation Days, 5 Sick Days, 3 Personal Days)
• Retirement Package – 401(K) Up to 5% Match
• Mileage Reimbursement - $0.62 per mile
• Autonomy

Responsibilities

• Monday – Friday (8:00 am-4:30 pm)
• Service Area (St. Croix, Pierce, and Dunn County)
• Visiting 3-5 Patient Per Day on Average
• Admissions
• Patient and Family Education
• Assessments of Patients
• Order Appropriate Therapies and Tests When needed
• Pain Management
• Comfort Care
• Coordinate with Physician When Needed
• Anticipate Changes and Educate the Patient, Family, and Caregivers on how to aid and Manage Symptoms Safely

Requirements

• Clear and Active NP License in the State of Wisconsin
• Hands-On Patient Care Experience
• Compassionate Care
• Nursing Experience Preferred, not Required
