    Job Title: QMHP-A MHSS Job Category: Human ServicesLocation: Hampton Roads Travel Required: Community-BasedReports to: Mental Health Skill-Building Services Supervisor/ Clinical Director Position Type: Part-time/Full-time/PRNDESCRIPTION: Seeking a Qualified Mental Health Professional – Adult (QMHP-A) to assist our agency with providing excellent, quality and impactful MHSS service within the Hampton Roads Area. Under the supervision of the Mental Health Skill-building Services Supervisor/Clinical Director, the QMHP-A will provide direct mental health services to individuals diagnosed with mental illness who are seeking to improve their ADLs and independent and symptom management skills. The QMHP-A will provide goal-directed training services as identified and documented in the Individualized Service Plan. QMHP-A will link individuals to appropriate resources, coordinate care with appropriate clinicians and complete all related documentation in a timely manner. These services are provided in accordance with state regulations and Together Lives Change policies.The QMHP-A will: 1. Implement recommendations given by the Department of Behavioral Health, Department of Human Services, and/or court systems.2. Maintain regular contact with the Referral Source and/or other service providers (i.e. PCP, Psychiatrist, Therapist, Case manager, family worker, MCO care coordinator, etc.) that are involved with the care of the individual as the individual’s progress status is being monitored by all providers. 3. Implement the Individual Service Plan (ISP).4. Provide a variety of interventions to assist the individual with meeting identified goals and objects.5. Maintain documentation identified by DMAS, MCO, and agency specific requirements.WORK ENVIRONMENT: The QMHP-A will spend the majority of the work week in the community with their client as activities are specifically designed to support the achievement of approved ISP goals. The QMHP-A will provide medication coordination; goal setting; linkage to services, advocacy, education and evaluation. Community interactions will include the Department of Behavioral Health, Department Human Services, local community services boards (CSBs), courts, schools, child support enforcement and other viable stakeholders/resources. Depending on case load and employment status, the QMHP-A will work approximately 15 hours per week per client and provide transportation assistance, if deemed necessary. Work schedule is flexible as QMHP-A and client will coordinate in the development of a schedule.SPECIFIC RESPONSIBILITIES:1. Coordinates scheduling of appointments, as needed.2. Provide a variety of interventions to assist with meeting identified goals and objectives.3. Provide goal-directed training and skill-building to individuals.4. Provide linkage to community-based services and supports.5. Principles of applying the standards of conduct.6. Ability to assist individuals with learning and developing new skills/behaviors.7. Responsible for the proper and timely documentation of all information required by the Department of Behavioral health and Developmental Services.8. Maintain regular and ongoing contact with referring agencies as well as other involved agencies.9. Actively coordinated and plans discharge and aftercare plans.10. Attend interagency meetings as necessary.11. Participates in supervision.12. Attend and participates in team meetings, orientation and in-service training.13. Provide and maintain updated credentials, CEU requirements, etc.14. Perform other duties as assigned.SKILLS:• Knowledge of Mental Health Skill-building Services policies and procedures.• Excellent time management skills.• Skills in providing community based services and supports.• Effective communication skills both verbal and written.• Knowledge of development and implementation of individualized service plans.• Collaborate with other health care provides or other individuals as needed for services.• Principles of applying the standards of conduct.• Ability to think critically and problem solve.• Able to be flexible.QUALIFICATIONS AND EXPERIENCE:• Bachelor’s degree in Human Services or related field (Social Work, Human Services Counseling, Psychology, Sociology, Child Development, Special Education, etc.). • Registered with the Virginia Board of Counseling as a QMHP-A.• A minimum of one (1) continuous year of demonstrated clinical full-time experience.
