🔍 Who We’re Looking For:

Registered Nurses (RNs) licensed in the state of Indiana

Minimum of 2 years experience in home health care (required)

Strong clinical assessment and critical thinking skills

Comfortable using EMR (Axxess experience a plus)

Professional, reliable, and compassionate

Committed to patient-centered care and ethical nursing practices

Conducting OASIS assessments, recertifications, and supervisory visits

Developing and implementing individualized care plans

Administering treatments and medications

Collaborating with multidisciplinary teams

Educating patients and caregivers

Maintaining thorough and timely documentation
