    Opportunity: Per-diem

    Pay rate: Starting at $35.00 per hr

    We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our team as a key member of our skilled nursing care facility. The ideal candidate will provide compassionate and high-quality nursing care to residents, ensuring their physical, emotional, and social well-being. The LPN will collaborate with the interdisciplinary healthcare team to deliver personalized patient-centered care and contribute to maintaining a safe and comfortable environment for residents.

    Responsibilities:

     

    Patient Assessment and Care:

    Conduct thorough ass

    essments of residents' health conditions and create individualized care plans.

    Administer medications and treatments as prescribed by physicians.

    Monitor and record vital signs, symptoms, and any changes in residents' conditions.

    Provide skilled nursing care, including wound care, catheter care, and other specialized procedures.

    Collaboration and Communication:

    Work closely with Registered Nurses (RNs), Certified Nursing Assistants (CNAs), and other healthcare professionals to coordinate and implement patient care plans.

    Communicate effectively with residents and their families, keeping them informed about the

    care plan, progress, and any changes in medical status.

    Documentation:

    Maintain accurate and up-to-date medical records, including nursing assessments, care plans, and progress notes.

    Document and report any incidents, changes in condition, or concerns promptly.

    Compliance and Quality Assurance:

     

    Adhere to all relevant nursing standards, policies, and procedures.

    Participate in quality assurance and performance improvement initiatives to enhance the overall quality of care.

     

    Patient Education:

    Educate residents and their families about their medical conditions, treatment plans, and preventive care measures.

    Promote health and wellness through patient and family education.

     

    Qualifications:

    Current and valid LPN license in OH.

    Minimum of (1) years of experience in skilled nursing or long-term care settings.

    Strong clinical skills and knowledge of nursing principles.

    Ability to assess, plan, implement, and evaluate patient care.

    Excellent communication and interpersonal skills.

    Compassionate and patient-centered approach to care.

    Knowledge of relevant regulations and compliance requirements.

     

    Education:

    Graduation from an accredited practical nursing program.

    Working Conditions:

     

    Fast-paced healthcare environment.

    Rotating shifts, including evenings, weekends, and holidays.

    Physical stamina and ability to lift/move patients when necessary.
