  • Responsibilities

    We are seeking a dedicated and compassionate LPN's & RN's to join our skilled nursing facility team. As an LPN/ RN, you will play a crucial role in providing high-quality care to our residents. The ideal candidate will have excellent clinical skills, a strong sense of empathy, and the ability to collaborate effectively with a multidisciplinary team.

    Responsibilities:

    Patient Care:

    Administer medications and treatments as prescribed by physicians.

    Monitor and assess residents' health status, promptly addressing any changes or concerns.

    Provide routine and emergency nursing care to residents as needed.

    Documentation:

    Maintain accurate and up-to-date medical records for each resident.

    Document nursing care and observations in accordance with facility policies and procedures.

    Collaborate with other healthcare professionals to ensure comprehensive care planning.

    Communication:

    Communicate effectively with residents, their families, and other members of the healthcare team.

    Report and discuss changes in residents' conditions with physicians and other healthcare providers.

    Participate in team meetings to discuss care plans and resident progress.

    Assessment and Monitoring:

    Conduct regular assessments of residents' physical, emotional, and mental well-being.

    Monitor vital signs and promptly address any abnormalities.

    Implement interventions to prevent complications and ensure resident safety.

    Collaboration:

    Work collaboratively with registered nurses, certified nursing assistants, therapists, and other healthcare professionals.

    Participate in interdisciplinary care conferences to discuss and adjust care plans as needed.

    Education and Support:

    Provide education and support to residents and their families regarding health conditions and care plans.

    Assist in the orientation and training of new staff members as needed.

    Qualifications:

    Education:

    Graduate of an accredited nursing program.

    Current Valid license in WA.

    CPR certification.

    Experience:

    Previous experience in a skilled nursing facility or similar healthcare setting is preferred.

    Familiarity with electronic health record systems is a plus.

    Strong clinical and critical thinking skills.

    Excellent communication and interpersonal skills.

    Ability to work collaboratively in a team environment.
