We are currently seeking a compassionate and reliable Transitional Aide to join our team.
Requirements:
MUST have FULL flexible availability (days, evenings, weekends as needed)
Reliable transportation
Ability to assist clients during transitional care periods
Strong communication and time-management skills
Dependable and professional demeanor
Responsibilities:
Provide support and assistance to clients during care transitions
Assist with daily living activities as needed
Follow care plans and agency policies
Communicate effectively with supervisors and care team
