Job Description

KEY RESPONSIBILITIES:

• Your primary goal is to drive the central intake / admissions process for several centers in the Chicagoland area.
• You will be 1 of 3 Centralized Admissions team members 
• You will would complete and oversee our PASRR Screening Program from Maximus.
• This will involve evaluating and assessing potential patients for clinical appropriateness and placements into one of our family of centers (SNF, ICF or SMHRF)
• You will provide clients (prospective residents, their families and referral sources) with facility and program specific information to help make it a smooth transition for all.

Qualifications

WHAT WE NEED FROM YOU:

• PRIOR EXPERIENCE IN AN ADMISSIONS DIRECTOR POSITION IN A SKILLED NURSING FACILITY (SNF) OR ICF IS A MUST.
• PREFERENCE GIVEN TO THOSE WITH EXPERIENCE IN BEHAVIORAL HEALTH
• Associates degree or higher is preferred
• RN or LPN  or Social Worker is preferred but not required if you have other relevant admissions experience
• Solid computer skills, including Point Click Care (PCC) is ideal
• Working knowledge of federal & state regulations and reimbursement (Managed Care, Medicare, and HMOs)
• Ability to 'work remote' (from home) should you wish. But you will be required to reside within the greater Chicagoland area.
